Terms & Conditions
Please read these Terms and Conditions carefully before accessing or using the services provided by Forefront Physiotherapy.
1. Acceptance of Terms: By accessing or using our services, you agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not access or use our services.
2. Location specific terms: Patients are responsible to abide by the rules and regulations of the club and or business where Forefront Physiotherapy operates a clinic from. By booking at St George's Hill Lawn Tennis Club you agree that you will abide by their non members rules and be signed in by acurrent member at reception on arrival. You also accept that nonmembers of St George's Hill Lawn Tennis Club have a treatment limit of 6 appointments and further treatment, should this be needed will be moved to another location as available.
3. Provision of Services: Forefront Physiotherapy provides physiotherapy services to patients in accordance with professional standards and regulatory requirements. We reserve the right to refuse service to any patient at our discretion.
4. Appointment Scheduling: Appointments for physiotherapy sessions are subject to availability. Patients are responsible for scheduling appointments in advance and arriving on time for their scheduled appointments. Late arrivals may result in shortened appointment times.
5. Payment and Billing: Patients are responsible for payment of all fees associated with physiotherapy services rendered. Payment is due at the time of service unless other arrangements have been made in advance. We accept payment by cash, cheque, private electronic funds transfer or direct health insurance transfer.
6. Cancellation and Rescheduling: To ensure appointment availability for all clients and to allow sufficient time to offer cancelled appointments to others, we operate the following cancellation policy:
Cancellations with more than 48 hours’ notice
Appointments cancelled with more than 48 hours’ notice will not incur any charge. Any deposit paid can be transferred to a future appointment.
Cancellations within 48 hours of the appointment
Appointments cancelled within 48 hours of the scheduled appointment time will result in the loss of the deposit, which is non-refundable.
Cancellations within 24 hours or non-attendance
Appointments cancelled within 24 hours of the scheduled appointment time, or failure to attend (non-attendance), will result in the full cost of the treatment being charged.
Health Insurance
Please note that missed appointment or late cancellation fees are not covered by private health insurers and remain the responsibility of the client.
We appreciate your understanding and cooperation, which allows us to maintain availability for all clients seeking treatment.
7. Confidentiality: We are committed to protecting the privacy and confidentiality of patients' personal information. Patient records are kept confidential and will not be disclosed to third parties without the patient's consent, except as required by law.
8. Limitation of Liability: Forefront Physiotherapy shall not be liable for any direct, indirect, incidental, consequential, or punitive damages arising out of or relating to the use of our services.
9. Indemnification: Patients agree to indemnify and hold harmless Forefront Physiotherapy, its affiliates, and their respective officers, directors, employees, and agents from any and all claims, liabilities, damages, and expenses arising out of or related to the patient's use of our services.
10. Governing Law: These Terms and Conditions shall be governed by and construed in accordance with the laws of England & Wales. Any disputes arising under these terms shall be subject to the exclusive jurisdiction of the courts of England & Wales.
11. Changes to Terms and Conditions: We reserve the right to update or revise these Terms and Conditions at any time without prior notice. Any changes will be posted on our website, and continued use of our services constitutes acceptance of the updated terms.
