Insurance Providers we work with

We work with a wide range of Health Insurance providers to offer our clients the most from their policies.

To use your insurance we must be made aware at the point of booking by emailing admin@forefrontphysiotherapy.co.uk and require the following information in order to submit your claim for treatment:

• Name of Insurance Provider

• Membership Number & Authorisation Number

• Number of Treatments authorised by your Provider

• Excess on your insurance policy

Note: Failure to provide notification and the above details prior to an appointment will result in the appointment being charged to the patient directly.
